At Invested Health Center, we believe that high-quality, affirming healthcare should be accessible to everyone—regardless of income, insurance status, or background. That’s why we offer a Sliding Scale Discount Program based on your household size and income.
A sliding scale means the cost of your care is adjusted based on your ability to pay. The lower your income, the lower your out-of-pocket cost for eligible services. This program ensures you can still receive essential healthcare—even if you’re uninsured, underinsured, or facing financial hardship.
Who Can Qualify?
You may be eligible if you:
- Are uninsured or underinsured
- Are not currently enrolled in Medicaid or other full-coverage health programs
- Have a limited income based on federal poverty guidelines
We welcome all individuals regardless of immigration status, gender identity, or sexual orientation.
How to Apply
To determine your eligibility and discount level, we’ll need:
- A completed Sliding Scale Application (available online or at our front desk)
- Proof of income (recent pay stub, W-2, tax return, or unemployment documentation)
- Proof of household size (if applicable)
Our team will review your application confidentially and apply any eligible discounts to your visit.
Services Covered
The sliding scale may apply to:
- Primary care visits
- STI & HIV testing and treatment
- PrEP consultations
- Wellness and chronic disease management
- Select laboratory tests
Some elective and all aesthetic services are excluded.
Nominal Fee – Fixed fee that does not reflect the true value of the service(s) provided and is considered nominal from the perspective of the patient. The nominal fee is approved by the board of directors to be collected at the time of service.
